Describe the collaborative / corroborative history obtained.
What are the role of carers and/or family members in the person’s life? How do carers / family members support the person?
How have health professionals worked with the carers or family members?
Diagnosis

What diagnosis has the medical practitioner given the person?
What is the prevalence of this disorder/disease in Australia?
What are the risk and/or predisposing factors for people generally developing this particular illness?
What are the most common comorbidities (pre-existing and/or developing) related to this disorder/disease in the general population?
Common Interventions and Recovery Plan

What are the most common interventions (physical, psychological, pharmacological, social) used to help people with this particular diagnosis?
What are the main interventions and/or services and/or support this person and his/her carers are receiving? What is the rationale for these interventions? Did the person choose these interventions? If not, why not?
If the consumer is on medications, what are these medications? Has the person experienced any side-effects? As a Registered Nurse, what can you do to help the person minimise these side-effects?
How are different members of the multidisciplinary team (e.g. Doctor, Allied Health professional/s) and/or support groups and/or community organisations supporting the person during their recovery?
To date, what are the outcomes of the person recovery plan?
Engagement

Explain and justify how you, as a Registered Nurse working in your field, engaged with the person and their carer or family members to support the person during their experience of the healthcare system.

Reflection

Reflect on the learnings that you take away from this unit/ Course and if it has has helped you to develop your specialist professional knowledge, skills and attitudes.
